Position Overview
As an LPN in a long-term care setting, you provide ongoing nursing care to residents, focusing on comfort, function, and decline management. You work within an interdisciplinary care team to support daily living and care plans.
The role emphasizes chronic condition management, safety, and coordination with families and clinicians within a residential care environment.
Responsibilities- Deliver routine nursing care and monitor residents' conditions
- Administer medications and document administration
- Assess changes in health status and report to the supervising RN
- Assist with daily activities and rehabilitation activities as directed
- Coordinate with physicians, therapists, and social workers
- Document care plans, progress, and family communications in the EHR
- Support safety, fall prevention, and infection control measures
Education:
Diploma or associate degree in nursing
Licensure:
Active LPN license in the state
Certifications:
- BLS certification usually required
Experience or Skills:
Experience in long-term care or geriatric settings with care coordination skills
Experience- Experience providing nursing care in LTC facilities
- Experience with documentation in the EHR and care planning
- Experience with wound care or chronic disease management
Residential care setting with a stable team and regular shifts. Documentation in the EHR and frequent interdisciplinary communication are standard.
