Location: West Lincoln
Overview
Registered Nurse, RN Hospice (Field) – PRN – provides nursing and supportive care for terminally ill patients and their families. Manages a caseload of patients/caregivers as assigned.
Responsibilities- Assessment:
Collect pertinent data, recognize findings, and attend to families at the time of patient’s death. - Coordination of Care:
Plan and evaluate care collaboratively with appropriate disciplines for assigned patients. - Diagnosis:
Analyze assessment data to determine actual or potential diagnoses, problems, and issues. - Evaluation:
Evaluate progress toward attainment of goals and outcomes, and report data and outcomes to physicians or other disciplines. - Implementation:
Implement and supervise care delivered by other team members in alignment with the plan of care and approved safety and infection control standards. - Knowledge:
Actively pursue required knowledge and skills through orientation activities, unit‑based in‑service programs, current literature, and new clinical learning opportunities. - Outcomes Identification:
Identify expected outcomes for an individualized patient plan. - Planning:
Develop a plan that prescribes strategies to attain expected, measurable outcomes. - Team Meetings:
Prepare for and participate in interdisciplinary team meetings. - Relationships:
Establish effective working relationships with peers, physicians, and other members of the health care team; identify and confer with appropriate resources regarding patient care decisions. - Documentation:
Submit required documentation on a timely basis. - Teaching:
Employ teaching strategies to promote health and a safe environment. - Reimbursement:
Verify reimbursement sources at admission and provide information regarding reimbursement for services to patients/clients and families. - On‑call Rotation:
Participate in on‑call rotation after office hours, weekends, and holidays.
- Minimum of one year of nursing experience.
- Registered Nurse licensed in Illinois by the Illinois Department of Financial and Professional Regulation.
- BLS or CPR certification within 90 days of hire.
- Valid driver’s license and automobile insurance.
- Excellent communication and customer service skills.
- Comfortable working with death and dying, and minimal supervision.
- Strong teamwork and relationship skills.
- Hourly compensation range: $37.23 – $57.71, pay based on experience.
