Shift: Overnight, 7:00 pm – 7:00 am
Setting: long-term care environment (LTC)

This travel LPN role is in a long-term care setting serving residents in a nursing-needs population. The assignment runs as a temporary travel position with consistent overnight coverage. You’ll work with an interdisciplinary team to deliver high-quality resident care, document appropriately, and maintain compliance with care plans and facility policies.

Responsibilities
  • Administer prescribed medications and therapies per care plans and physician orders
  • Monitor residents’ health status, assess for changes, and report to the supervising RN
  • Assist with activities of daily living and mobility, ensuring safety and comfort
  • Perform routine procedures such as wound care, dressing changes, and vital signs
  • Document care provided clearly and accurately in the resident record
  • Collaborate with the interdisciplinary team to maintain comprehensive, patient-centered care
Requirements
  • Active LPN license in Vermont in good standing
  • Ability to work the overnight shift (7:00 pm–7:00 am) in a long-term care setting
  • Willingness to work a travel assignment in the specified location
  • Strong communication and teamwork skills; commitment to resident safety and quality of care
