• Provide direct care to our residents, including administering medications, wound care, and vital signs management
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Assist with admissions, transfers, and discharges, ensuring a smooth transition for our residents
• Maintain accurate and detailed records of patient care and communications
• Collaborate with other healthcare professionals to ensure comprehensive care

• Provide high-quality patient care, consistently meeting and exceeding industry standards
• Participate in quality improvement initiatives, identifying areas for improvement and implementing changes
• Communicate effectively with patients, families, and staff to ensure seamless care transitions

• Participate in on‑call rotations, providing emergency care services as needed
