Registered Nurse (RN) - Intellectual and Developmental Disabilities

This RN specialty provides care coordination and medical management for individuals with intellectual and developmental disabilities (IDD). The role emphasizes accessible care, communication, and collaboration with family, caregivers, and community supports.

Care is delivered in clinics, community-based programs, and integrated settings to support meaningful outcomes and preventive health.

Responsibilities
  • Perform comprehensive health assessments tailored to IDD populations and track co-morbid conditions.
  • Coordinate preventive care, routine screenings, and management of chronic diseases.
  • Collaborate with behavioral health, therapists, and caregiving teams to implement care plans.
  • Educate patients, families, and caregivers on health maintenance, medications, and safety considerations.
  • Document care plans and progress in the EHR; advocate for patient-centered accommodations.
  • Monitor for communication barriers and adapt strategies to support understanding and consent.
  • Identify gaps in care and coordinate referrals to specialty services as needed.
  • Respond to acute changes and escalate concerns per clinical protocol.
Qualifications

Education:

RN with active US licensure; BSN preferred.

Licensure:
Current, unrestricted RN license to practice in the United States.

Certifications:

Certification in IDD care or pediatric/adult IDD-focused nursing preferred.

Experience or

Skills:

Experience in IDD care, community-based services, and care coordination; strong communication and caregiver engagement skills.

Experience
  • 2-4 years of nursing experience in IDD or community health settings.
  • Experience coordinating interdisciplinary teams and supporting transition between settings (clinic, home, group homes).
Preferred
  • IDD nursing certification; experience with behavioral health integration.
Work Environment

Roles commonly span clinics, residential settings, and community programs; teams include family/caregivers and support staff.

Documentation in EHR and flexible scheduling to support appointments, home visits, and care coordination activities.
