Bayada Home Health Care is hiring a Registered Nurse (RN) for home health visits in North Hartland, Vermont. The position is full‑time, ongoing, and staff.
OverviewAs an RN, you will be an integral member of a multi‑disciplinary health care team that provides skilled nursing and rehabilitative care to clients, enabling them to remain at home.
Responsibilities- Perform comprehensive nursing assessments and identify subtle changes in patient condition.
- Triage and manage urgent clinical situations independently in the home setting.
- Provide medication reconciliation, teaching, and safety monitoring.
- Carry out wound assessment, care, and documentation.
- Make real‑time clinical decisions and know when to seek escalation.
- Manage a dynamic visit schedule, prioritize urgent visits, and maintain productivity expectations.
- Use point‑of‑care documentation and technology efficiently.
- Deliver care in varied home environments with cultural sensitivity and respect for patient autonomy.
- Current RN license in good standing in Vermont.
- Graduation from an accredited nursing program (transcript or diploma).
- At least one year of recent hands‑on medical experience with adults/geriatrics in acute care, emergency, home health, hospice, ICU, med‑surg, sub‑acute or rehab settings.
- Strong head‑to‑toe assessment skills and experience with CHF, COPD, wound care, IV therapy, and post‑surgical care.
- Ability to work independently while escalating concerns appropriately.
- Strong problem‑solving skills in unpredictable home environments.
- Adaptable to changing patient needs and schedules; efficient with documentation.
Competitive weekly base pay $60–$65 per point, subject to qualifications and negotiation. Full‑time positions may qualify for a bonus opportunity up to $10,000. Pay is distributed weekly and scheduling is flexible.
