Home Health Nurse

This is a 13-week contract position.

Monday to Friday

Nursing with flexibility, trust, and purpose. Our Home Health RNs are trusted to use their clinical judgment while providing personalized care tailored to each patient's needs. You'll enjoy the balance of independence in the field with the support of a collaborative care team, allowing you to focus on quality care, continuity, and patient education without the constant pace of inpatient settings.

Responsibilities:

• Complete an initial assessment of patient and family to determine home care needs.
• Conduct a complete physical assessment and history of current and previous illness(es).
• Regularly re-evaluate patient nursing needs and initiate the plan of care.
• Develop care plans that establish goals based on nursing diagnosis.
• Provide direct patient care, administer medications and treatments as prescribed.
• Counsel patients and families and provide health care instructions.
• Identify discharge planning needs and coordinate patient care.
• Prepare clinical notes and communicate with physicians regarding patient care.
• Ensure necessary equipment and services are available for patient care.

Required Experience / Certifications / Licensure:

• Graduate of an accredited school of nursing.
• One (1) to two (2) years of acute care experience in an institutional setting preferred.
• Current licensure as a Registered Nurse in the state and CPR certification.
• Knowledge of OASIS and compliance with professional standards.
• Proficient observation and communication skills; able to lift and position patients.
