We are seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) or Physician Assistant with psychiatric experience to join our expanding behavioral health team for a full-time role located in Queens, NY
. Our providers become an integral part of the care team for residents in skilled nursing facilities who often have limited access to specialized psychiatric services. This is a mission-driven, full-time position where your clinical expertise directly improves the emotional health, behavioral stability, and quality of life of medically complex older adults. Providers with experience caring for patients with traumatic brain injuries (TBI) are highly valued, as many residents present with neuropsychiatric conditions requiring specialized assessment and treatment.

Requirements
  • Active and unrestricted New York State clinical license.
  • Board Certified PMHNP-BC or Physician Assistant with significant psychiatry experience.
  • Active DEA registration.
  • Provide comprehensive psychiatric consultations, diagnostic evaluations, and medication management.
  • Evaluate adult and geriatric residents experiencing depression, anxiety, dementia, and chronic psychiatric illness.
  • Assess residents with traumatic brain injury (TBI) and neurological conditions (strongly preferred).
  • Develop individualized treatment plans addressing cognitive, behavioral, and medical needs.
  • Initiate, monitor, and adjust psychotropic medication regimens for medically complex patients.
  • Perform ongoing psychiatric follow-up visits to evaluate treatment response and behavioral stability.
  • Collaborate closely with facility physicians, nursing leadership, therapists, and social workers.
  • Educate residents, caregivers, family members, and facility staff regarding diagnoses and interventions.
  • Maintain accurate, timely, and compliant documentation within electronic medical record systems.
  • Strong background in psychiatric assessment, psychopharmacology, and interdisciplinary healthcare.
