• Administer medications and treatments according to physician orders and facility policy
• Monitor residents’ health status, document changes, and elevate concerns to the supervising RN
• Assist with activities of daily living and provide basic nursing care as needed
• Maintain accurate and timely documentation in resident records (EMR/MAR) and update care plans
• Respond promptly to call lights, ensuring resident safety and comfort
• Collaborate with the interdisciplinary team to develop and implement individualized care plans

• Active Licensed Practical Nurse (LPN) license
• Ability to work across all shifts; 40 hours guaranteed per week
• Experience in long-term care or geriatric nursing is preferred
• Strong documentation and communication skills; ability to work in a fast-paced setting
• Commitment to resident safety, dignity, and high-quality care
