WhoWe're Seeking:
Are you ready to make a meaningful difference in the lives of individuals and families facing end-of-life challenges? Our Hospice Care team is seeking a dedicated Registered Nurse to deliver personalized, one-on-one care in the comforting setting of patients' homes.
ScheduleWe have multiple schedule variations available for this role.
Saturday & Sunday 8 hour days
Friday, Saturday & Sunday or Saturday, Sunday & Monday 8 hour days
Please note all schedules will include alternating weekend on call expectations
Your Responsibilities:As a member of our dedicated team, you'll:
- Provide compassionate end-of-life care, ensuring the physical and emotional well‑being of patients and their families.
- Deliver expert pain and symptom management, offering comfort and support during challenging times.
- Collaborate closely with our interdisciplinary team, local physicians, and community providers to optimize the quality of life for patients.

- Vermont Licensed RN with a passion for hospice and palliative care.
- Minimum of two years clinical med‑surg and acute care nursing experience required. Minimum of one year of Hospice experience required. Home care experience and/or previous case management experience preferred.
- Valid VT driver's license and reliable vehicle with automobile insurance is essential for home visits.
