• Assess and monitor individuals with intellectual and developmental disabilities, noting changes in health status and behavior.
• Administer medications and treatments per physician orders; document interventions in the charting system.
• Develop, implement, and evaluate individualized nursing care plans in collaboration with an interdisciplinary team.
• Provide patient and family education on condition management, medications, safety, and follow-up care.
• Monitor safety, respond to clinical changes, and apply de‑escalation or crisis intervention techniques as needed.
• Communicate effectively with case management and other health professionals to coordinate care and discharge planning.

• Active Registered Nurse license in Vermont (in good standing)
• Nursing degree (Associate or Bachelor’s) from an accredited program
• Current Basic Life Support (BLS) certification
• 1+ year of clinical experience in intellectual/developmental disabilities or community mental health preferred
• Ability to work the day shift in a travel assignment with guaranteed 40 hours per week
• Strong documentation, communication, and teamwork skills; ability to collaborate in a multidisciplinary environment
