• Provide nursing care to residents under supervision of a registered nurse
• Administer medications and treatments per physician orders and facility policies
• Monitor resident status (vital signs, pain, hydration, nutrition) and report changes promptly
• Assist with activities of daily living and mobility; promote safety and fall prevention
• Document care accurately in the resident’s chart; communicate with the interdisciplinary team
• Uphold infection control practices and resident rights; maintain confidentiality and professional boundaries

• Active Vermont LPN license in good standing
• LPN diploma or certificate
• Prior long-term care experience preferred
• Ability to work all shifts and meet the guaranteed 40 hours per week
• Strong communication and teamwork skills; comfortable collaborating with an interdisciplinary team
• Proficiency with basic electronic charting and documentation
