DescriptionSometimes it takes only one person to make a difference in a life. As the Care Navigator Case Manager for children's services, you could be that person. The Care Navigator Case Manager leads service planning and care coordination navigation for children and families receiving mental health services, including managing referrals, providing primary care screening and monitoring and serving as a primary liaison with community partners.

- An Associate's Degree and two years experience and/or Bachelor's Degree.
- The ability to establish and maintain supportive relationships with children, youth and families.
- Knowledge on how to provide holistic and therapeutic case management services.
