Physical Therapist

Join Our In-House Therapy Team at The Center for Living and Rehab!

We are currently seeking a PRN Physical Therapist (PT) to join our in-house rehab team. At The Center for Living and Rehab, we take pride in delivering high-quality, resident-centered care in a supportive and team-oriented environment.

• Evaluate residents and develop individualized physical therapy treatment plans
• Deliver skilled therapy services to help improve mobility, strength, and overall function
• Monitor progress and adjust treatment plans based on resident response
• Educate residents, caregivers, and staff on therapy goals and safety techniques
• Document evaluations, treatment sessions, and outcomes accurately and timely using electronic medical records
• Collaborate with interdisciplinary team members to support resident care plans

• Graduate of an accredited Physical Therapy program
• Active Vermont Physical Therapist license (required)
• Previous experience in SNF or long-term care preferred
• Strong communication, organizational, and clinical skills
• Experience with EMR systems preferred
