Medical Social Worker (Home Health & Hospice – Travel Contract)

Contract - 8 Weeks

Shift: 8-hour Day Shifts

Schedule:

Monday–Friday (home hospice). Occasional cross-coverage for adult home health, inpatient hospice, and community hospice as needed.

On-Call:
Rare coverage, if needed

We are seeking an experienced Medical Social Worker (MSW) to support patients and families across home hospice, community hospice, and inpatient hospice settings. This role focuses on end-of-life care, psychosocial assessment, care planning, and interdisciplinary collaboration.

You will primarily be assigned a geographic area and will support patients in home, SNF/ALF, and inpatient hospice settings as needed.

Terminally ill patients (life expectancy of 6 months or less)

Adults primarily; occasional pediatric patients in the home setting

Provide psychosocial assessments, counseling, and care planning for hospice patients and families

Participate in goals-of-care discussions and interdisciplinary team meetings

Support complex symptom management and end-of-life planning

Coordinate community resources and services

Provide caregiver education and emotional support

Maintain timely and accurate clinical documentation

Assist with coverage across home hospice, inpatient hospice, and community hospice as needed

Home hospice nurses carry caseloads of approximately 12–14 patients

Overnight inpatient shift: up to 11 patients

Advanced Practice Provider support available

Net Smart (training provided during orientation)

Facility is converting to Epic

Recent Epic experience is required (within the past 24 months)

Education:

Master’s Degree in Social Work (MSW)

Experience:

Minimum 2+ years of relevant experience (3+ years preferred)
