Location: Town of Vermont
A healthcare team in Saint Johnsbury, Vermont is seeking a Primary Care Nurse Practitioner (FNP or AGNP) to provide on-site care for residents in skilled nursing and long-term care facilities
. This full-time role offers the opportunity to build meaningful patient relationships while managing complex chronic conditions in a collaborative care environment.
Providers will deliver comprehensive primary care, perform assessments, manage treatment plans, and coordinate care with facility staff and interdisciplinary teams to improve health outcomes for older adults.
Key Responsibilities- Provide on-site primary care to residents in skilled nursing and long-term care facilities
- Perform physical exams and comprehensive medical assessments
- Diagnose and manage chronic and acute conditions
- Order and interpret laboratory tests and diagnostics
- Prescribe and monitor medications
- Collaborate with nursing staff and care teams to coordinate treatment plans
- Active Vermont Nurse Practitioner license
- Experience in long-term care, geriatrics, or skilled nursing settings preferred
- Strong clinical judgment and communication skills
- $150,000 – $200,000 annual salary
- On-site care in skilled nursing / long-term care facilities
