Family Nurse Practitioner

As a Family Medicine Nurse Practitioner at Community Health, you’ll be part of a collaborative, multidisciplinary team delivering full-spectrum care to patients of all ages.

The Castleton practice uniquely provides the community with access to Primary Care, Behavioral Health and Care Management services for the whole family. Primary Care services include on-site Behavioral Health providers, Social Workers, and Care Management, along with preventive programs like diabetes management and education. Castleton Express Care, located within the same facility as Community Health Castleton, is open seven days a week for the treatment of minor illnesses and injuries for adults and children.

• Flexible 4-day work week
• Outpatient-only care model
• Minimal call – approx. 3 weeks per year (5:00 PM–8:00 AM coverage)
• Team-based care with embedded support staff
• Opportunities for mentorship and clinical precepting
