Location: Town of Vermont
Woodridge Rehabilitation & Nursing at Central Vermont Medical Center (CVMC) seeks an experienced Nurse Practitioner or Physician Assistant to join our interdisciplinary team. You will care for a complex adult and geriatric population in an inpatient rehabilitation setting, balancing continuity of care with acuity.
You will lead rounds, manage a panel of 6–8 patients daily, collaborate with physicians, nurses, rehab therapists and care managers, and support safe discharges back to primary care.
