In-Home Health - Clinician NP​/PA (Per Diem)- Bennington VT

Overview

Signify Health, part of CVS Health®, is looking for a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations. In this role, you will connect with patients in their homes, conduct health assessments, and help bridge gaps in care.

• Conduct a 45‑minute home visit using an iPad and clinical workflow.
• Review medication and medical history.
• Perform a physical evaluation.
• Order and perform diagnostic tests (e.g., diabetic eye exams, spirometry) when ordered by the patient’s health plan.
• Provide diagnostic and preventive services such as spirometry, DEE, and bone density screening.
• Conduct virtual visits as required.
• Travel locally and within licensed states as needed.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).
• Ability to cover primary locations:
  Boston, MA.
• Ability to meet the anticipated weekly hours of 12.

Anticipated weekly hours: 12. Time type: part‑time. Pay range: $41.85 – $90.13 per hour, depending on experience, education, geography, and other factors.
