Position: Travel Nurse - Rn - Outpatient Clinic - $1790 / Week
Location: Town of Vermont

Overview

Center Well Home Health, part of Humana Inc., is seeking a Home Health Registered Nurse to deliver compassionate, patient-centered care in the comfort of patients’ homes. In this role, you’ll perform comprehensive assessments, develop and implement individualized care plans, administer medications and treatments, and educate patients and caregivers on managing chronic conditions. You’ll collaborate closely with an interdisciplinary team, use innovative tools and electronic documentation, and help seniors and adults remain safe and independent at home while growing your professional skills in a supportive, team-focused culture.

Responsibilities
  • Lead a team of direct care providers to ensure appropriate execution of medications and treatments, documentation, family teaching, care planning and patient care conferencing in compliance with the Health Campus Policies and Procedures.
  • Utilize the nursing process in delivering patient care and ensure continuity of care from admission through discharge and return to family and community.
  • Visit residents on assigned unit daily to observe and evaluate each resident’s physical and emotional status and notify the charge nurse, the physician, and/or the Director of Nursing when there is a change in a resident’s condition.
  • Identify resident problems and emergency situations and initiate immediate lifesaving measures in the absence of a physician.
  • Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc. as necessary.

Qualifications
  • Must have and maintain a current, valid state RN license.
  • Current, valid CPR certification required.
  • Valid RN licensures as required by state regulations.
  • Valid CPR certification (ACLS preferred).
  • CRRN certification preferred.
  • One year of experience in a rehabilitation hospital setting is preferred.

M-F; 8 hour shifts (on call rotating)
