Job Description - RN Case Manager (7420‑1259)

Ashley Regional Medical Center

• Coordinate with physicians, staff, community resources and facilities regarding patient care.
• In conjunction with physicians, develop plans of care and discharge plans, monitor all clinical activities, make recommendations for alternative levels of care, identify cost‑effective protocols, and develop guidelines for care.
• Communicate, collaborate, and coordinate with team members, patients, and families to provide quality patient care and ensure positive patient outcomes.
• Perform utilization reviews using guidelines and policies/procedures to determine appropriate levels of care and readiness for discharge.
• Ensure continuity of care through communication during rounds and written documentation.
• Maintain compliance with CMS standards and state/federal regulatory requirements, including The Joint Commission (TJC) accreditation standards and HIPAA.
• Address staff, physician, patient, and family concerns immediately and initiate appropriate corrective steps.

• Must be a licensed registered nurse (RN) in the State of Utah.
• At least one year of hospital‑based case management experience preferred.
• Certification in Case Management or a Bachelor of Science in Nursing (BS/BSN) preferred.
