Registered Nurse (RN) Case Manager – Home Health & Hospice $36–$39/hour + Mileage Reimbursement + Cell Phone Reimbursement | Full-Time | Benefits Make a Difference Where It Matters Most Rocky Mountain Care Home Health & Hospice is looking for a compassionate, dependable Registered Nurse (RN) Case Manager to join our growing team in Vernal, Utah.

You'll be part of a collaborative clinical team while providing care throughout the Vernal area.

What You'll DoAs an RN Case Manager, you will:

Assess and monitor patient conditions and changing care needs

Develop, implement, and update individualized care plans

Administer medications and treatments as ordered

Provide skilled nursing care, including wound care and IV therapy

Collaborate with physicians and members of the interdisciplinary care team Maintain accurate and timely clinical documentation

Coordinate patient care and communicate changes in condition appropriately

Provide compassionate support and education to patients and their families

Help ensure patients receive quality care in the comfort of their own homes

What We're Looking For Current, unrestricted Registered Nurse (RN) license

Hospice nursing experience preferred

Home Health nursing experience accepted

Strong clinical judgment and assessment skills

Excellent communication and organizational skills

Ability to work independently while collaborating effectively with a care team Compassionate, patient-centered approach to care
