Registered Nurse (RN) Case Manager – Home Health & Hospice

Rocky Mountain Care Home Health & Hospice is looking for a compassionate, dependable Registered Nurse (RN) Case Manager to join our growing team in Vernal, Utah.

As a Home Health & Hospice RN, you'll have the opportunity to build relationships with patients and families, coordinate care with physicians and other healthcare professionals, and make a meaningful difference in the lives of the people you serve.

You’ll be part of a collaborative clinical team while providing care throughout the Vernal area.

What You’ll Do
  • Assess and monitor patient conditions and changing care needs
  • Develop, implement, and update individualized care plans
  • Administer medications and treatments as ordered
  • Provide skilled nursing care, including wound care and IV therapy
  • Collaborate with physicians and members of the interdisciplinary care team
  • Maintain accurate and timely clinical documentation
  • Coordinate patient care and communicate changes in condition appropriately
  • Provide compassionate support and education to patients and their families
  • Help ensure patients receive quality care in the comfort of their own homes
What We're Looking For
  • Current, unrestricted Registered Nurse (RN) license
  • Hospice nursing experience preferred
  • Home Health nursing experience accepted
  • Strong clinical judgment and assessment skills
  • Excellent communication and organizational skills
  • Ability to work independently while collaborating effectively with a care team
  • Compassionate, patient-centered approach to care
