Staff, 40 hours per week, 8‑hour shift, Ongoing.

Deliver skilled, patient‑centered care in patients’ homes, collaborating with an interdisciplinary team. Responsibilities include patient assessments, medication management, care planning, education of patients/families, critical thinking, EMR documentation, independent work, and travel within assigned territory.

• Active Registered Nurse license in the state of practice.
• Minimum one year of RN experience with clinical expertise for quality patient care.
• Current driver’s license and ability to drive approximately 20% of the day to client locations.
