Talented Medical Solutions is seeking a compassionate and motivated Physical Therapist to join a collaborative care team in Vernon, Connecticut. In this role, you’ll evaluate patients, develop personalized therapy plans, and guide individuals of all ages through their rehabilitation journeys. You’ll play a vital role in helping patients regain mobility, improve strength, and return to daily living with confidence.

Key Responsibilities:

Perform physical therapy evaluations and treatment planning

Lead therapeutic exercises and provide manual therapy

Document patient progress and outcomes

Educate patients and families on care plans and home exercises

Work closely with other care providers to ensure comprehensive care

Active Physical Therapist license in Connecticut (or willing to obtain)

Graduate of an accredited Physical Therapy program

BLS certification (required)

At least 1 year of recent experience in a clinical setting

Strong interpersonal skills and a patient-centered mindset
