Nurse Practitioner- PRN

Overview

Matrix Medical Network provides comprehensive in‑home health assessments to improve quality of care and close multiple care gaps in a single visit.

• Conduct adult/geriatric assessments, including medical history, diagnosis, treatment planning, health education, physician referrals, case management referrals, and follow‑up, while documenting in accordance with Matrix guidelines and protocols.
• Administer and order point‑of‑care and laboratory testing as needed.
• Collaborate with physicians, case managers, social workers, family members, key caregivers and ancillary medical personnel.
• Work with the Primary Care Physician on patient education and follow‑up as necessary.
• Provide services at the member’s place of residence, including homes and skilled nursing facilities.

• Flexibility:
  Visits scheduled based on your availability (Sunday - Saturday, 8 am-7 pm) and can be performed during the day, evening, or weekend.
• Competitive pay:
  Earn income based on the number of assessments completed. Potential earnings $1,800-$11,500 per month, dependent on assessment rate ($90-$115) and state of residence.

• Master’s degree or commensurate experience with satisfactory completion of NP licensure.
• Current NP licensure in state of practice, including prescription authority or the ability to obtain it.
• Board certification from AANP, AACN or ANCC in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency or Acute Care specialty.
• Current BLS, ACLS or CPR certification.
• One year of NP experience preferred; new graduates encouraged.
• Valid state driver’s license for the state of residence.
• Automobile insurance coverage required for the state of residence (or ability to obtain).
• Strong computer skills and familiarity with employee health/medical record software.
• Excellent verbal and written communication skills.
