Overview
Matrix Medical Network provides comprehensive in‑home health assessments to improve quality of care and close multiple care gaps in a single visit.
Responsibilities- Conduct adult/geriatric assessments, including medical history, diagnosis, treatment planning, health education, physician referrals, case management referrals, and follow‑up, while documenting in accordance with Matrix guidelines and protocols.
- Administer and order point‑of‑care and laboratory testing as needed.
- Collaborate with physicians, case managers, social workers, family members, key caregivers and ancillary medical personnel.
- Work with the Primary Care Physician on patient education and follow‑up as necessary.
- Provide services at the member’s place of residence, including homes and skilled nursing facilities.
- Flexibility:
Visits scheduled based on your availability (Sunday - Saturday, 8 am-7 pm) and can be performed during the day, evening, or weekend. - Competitive pay:
Earn income based on the number of assessments completed. Potential earnings $1,800-$11,500 per month, dependent on assessment rate ($90-$115) and state of residence. - Mileage reimbursement.

- Master’s degree or commensurate experience with satisfactory completion of NP licensure.
- Current NP licensure in state of practice, including prescription authority or the ability to obtain it.
- Board certification from AANP, AACN or ANCC in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency or Acute Care specialty.
- Current BLS, ACLS or CPR certification.
- One year of NP experience preferred; new graduates encouraged.
- Valid state driver’s license for the state of residence.
- Automobile insurance coverage required for the state of residence (or ability to obtain).
- Strong computer skills and familiarity with employee health/medical record software.
- Excellent verbal and written communication skills.
