- Provide skilled nursing care to clients in home and community‑based settings ensuring delivery of outcome‑based, high‑quality, cost‑effective care.
- Collaborate with all team members to achieve exceptional outcomes.
- Maintain a client record utilizing the Agency’s standard for documentation.
- Make follow‑up visits to provide skilled care to assess disease progress, evaluate response to treatment, and provide support to patient/family using teaching principles to promote and restore health and prevent disease and disability; coordinate care with providers.
- Carry out the plan of care.
- Teach patients and reinforce client/caregiver teaching and education.
- Refer, collaborate, and coordinate all client care services; consult with physician regarding client needs and plan of care.
- Communicate and confer with Nurse Care Coordinator and all involved in the patient’s care.
- Direct LPNs in carrying out the established plan of care; direct Home Care Aides and supervise plan of care.
- Refer client/family to other community resources as needed.
- Attend and participate in in‑service education program.
- Education:
Graduate of an Associate’s Degree or diploma program in nursing. - Experience:
One year nursing experience or one year nursing experience in a specialty. - Licensure:
Registered nurse with a license to practice in the state of CT.
- Education:
Graduate of an NLN approved BSN program. - Experience:
Medical/surgery or home care experience.
