On Call Visiting Nurse – MMH-8 Keynote Drive (10628)
Shift Details- On call position, 4 pm – 8 am, rotating with a second on‑call RN.
- Requires hospice admission visits daily.
- Assumes responsibility for evening, night, weekend, and holiday on‑call duty as scheduled and consistent with Agency policies and procedures.
- Assumes primary responsibility for total region census during the assigned shift, including the assessing, planning, implementing, and evaluation phases of the nursing process.
- Maintains regular communication with nurse supervisors, attending physicians, and case managers concerning patient/family care needs and responses to interventions.
- Responds to medical or emotional crises and makes home visits to provide direct care as appropriate.
- Reviews referrals and initiates contact with clients and families.
- Conducts comprehensive nursing assessments based on referral data, client/family interviews, and clinical observations of patients’ health status, demonstrating clinical expertise in symptom management.
- Develops care plans in collaboration with clients and all disciplines, documents service needs, discharge planning, short‑ and long‑term goals, and resource utilization; modifies plans as conditions change.
- Demonstrates understanding of payer information to maximize care planning.
- Initiates and maintains client records using the Agency’s standard documentation systems.
- Makes follow‑up visits to assess disease progression, evaluates treatment response, provides support to clients/families using teaching principles, and coordinates care with providers.
- Teaches patients and reinforces caregiver education, preparing and supporting families regarding signs and symptoms as appropriate.
- Refers, collaborates, and coordinates all client care services; consults with physicians regarding client needs and care plans.
- Directs LPNs in carrying out established care plans.
- Directs Home Care Aides and supervises the care plan.
- Recommends community resources to clients/families as needed.
- Attends and participates in in‑service education programs.
- Seeks consultation and guidance from supervisors.
- Completes reporting requirements in a timely manner.
- Actively participates in the improvement process (H3W) to identify team service line needs and development.
- Performs other duties as assigned.
- Provides skilled nursing care to clients in their homes or community health settings, ensuring the delivery of outcome‑based, high‑quality, cost‑effective care.
- Education:
Graduate of an Associate’s or diploma program in nursing education required; graduate of an NLN‑approved BSN program preferred. - Experience:
One year of nursing experience or one year in a specialty (medical/surgery or home care) required. - Licensure:
Registered Nurse with a valid license to practice in the state of Connecticut required. - Knowledge, Skills & Abilities:
Computer skills necessary.
