Location and Shift

Location: 8 Keynote Drive (10628)
Shift: On‑call RN for Friday 4:30 pm–Saturday 8 am and Saturday 4:30 pm–Sunday 8 am

Work where every moment matters. As an On‑Call Visiting Nurse for Hartford Health Care at Home, you provide high‑quality, person‑centered care to clients in their homes or community settings. Your role supports a comprehensive network that has been delivering home‑care services in Connecticut for more than 115 years.

• Assume responsibility for evening, night, weekend, and holiday on‑call duty as scheduled, in accordance with Agency policies and procedures.
• Manage the total region census during the assigned shift, including assessment, planning, implementation, and evaluation phases of the nursing process.
• Maintain regular communication with Nurse Supervisors, attending physicians, and case managers regarding patient and family care needs and responses to interventions.
• Respond to medical or emotional crises and perform home visits to provide direct care as appropriate.
• Review referrals and initiate contact with clients and families.
• Conduct complete nursing assessments based on referral data, client/family interview, and clinical observation of patient health status.
• Demonstrate clinical expertise in symptom management and develop plans of care in collaboration with clients and interdisciplinary teams.
• Document and modify plans of care as conditions change, ensuring documentation meets Agency standards.
• Utilize payer information to maximize care planning and resources.
• Make follow‑up visits to assess disease progress, evaluate responses to treatment, and provide patient education and support.
• Teach patients and reinforce caregiver education, preparing families regarding signs and symptoms as needed.
• Refer, collaborate, and coordinate all client care services; consult with physicians about client needs and care plans.
• Direct LPNs and Home Care Aides in carrying out established plans of care.
• Refer clients/families to community resources as needed.
• Attend and participate in in‑service education programs; seek consultation and guidance from supervisors.
• Complete reporting requirements promptly.
• Participate actively in the H3W improvement process to identify service line needs and development.
• Perform other duties as assigned.
• Provide skilled nursing care to clients in their homes or community settings, ensuring delivery of outcome‑based, high‑quality, cost‑effective care.

Education: Graduated from an Associate’s or diploma nursing program (required). Graduate of an NLN‑approved BSN program preferred.

Experience: One year of nursing experience or one year in a specialty such as medical/surgery or home care (required).

Licensure, Certification, Registration: Registered Nurse licensed to practice in the state of Connecticut.

Knowledge, Skills, and Abilities: Proficient computer skills and ability to manage documentation, communication, and clinical decision‑making.
