Per Diem Home Health Nurse Practitioner in Vernon Rockville, CT

This per diem opportunity allows Nurse Practitioners to work independently, conducting focused home health risk assessments for Medicare Advantage members throughout Windham County, CT, including areas such as Windham (Willimantic), Killingly, and Mansfield. Choose your own schedule and work in a variety of patient care settings without the need to develop treatment plans or prescribe medications.

• Flexible shifts available Monday through Sunday
• Create your own schedule
• Minimum weekly or bi-weekly hourly commitment

• Perform comprehensive annual home health risk assessments for Medicare Advantage plan members
• Evaluate each patients medical history, current medical conditions, and medication use
• Deliver education on managing medical issues and medication regimens
• Identify any gaps in care and provide actionable recommendations
• Document findings; no requirement to prescribe medications or create longitudinal care plans

• Active Nurse Practitioner license in Connecticut
• Board certification through AANP or ANCC
• Masters degree in Nursing (MSN) from an accredited program
• Current CPR certification (BLS)
• At least 6 months of recent experience as an NP preferred
