Hartford Health Care is seeking a Hospice Advanced Practice Registered Nurse to deliver patient-centered hospice care in community settings. The APRN will perform face-to-face visits for eligibility and recertification, collaborating with the IDT to ensure quality symptom management and goals of care.
Applicants should be an APRN (FNP/AGNP/Adult NP) licensed in Connecticut with 2–3 years of relevant experience, preferably in hospice, palliative care, geriatrics, or home-based care.
