Position Scope

This position will provide community support/case management services. Responsibilities will primarily consist of the provision of direct care, and will be mostly community based; generally consist of guiding, assisting and supporting individuals who have complex psychiatric, substance abuse, medical, social and legal issues to improve their daily living skills, and assist them in their recovery process. Hours may vary by program.

Responsibilities
  • Provides a full range of community support services which may include targeted case management, housing, as well as teaching skill development.
  • Completes timely rehabilitation assessment, reassessments and completes/updates recovery plans.
  • Engages clients and works in partnership to increase or maintain involvement in learning, motivation of change, working and social activities.
  • Pursues best practice outcomes for clients in continuing education, employment and socialization.
  • Develops relationships with clinical, educational, vocational and other social service agencies in order to effectively support clients in the community with increasing levels of independence.
  • Provides continuous assessments, modifies interventions as appropriate and completes documentation, of required paperwork based on funder/program/agency requirements.
  • Provides transportation to clients as appropriate utilizing personal vehicle for agency business.
  • Assist in the facilitation of self-help and advocacy support groups for program consumers.
  • Attends and participates in all required meetings.
  • Adheres to all Chrysalis Center, Inc.’s policies, procedures and standards at all times.
  • Performs other duties as assigned.

Bachelor’s degree plus one to two years of experience in mental health and substance abuse fields. Masters’ degree may be considered in lieu of general experience.

Qualifications
  • Bi-lingual Spanish, preferred.
  • Experience managing program delivery and providing residential and community case management services.
  • Proficiency managing Microsoft and other computer applications used for tracking data and reporting outcomes.
  • Easily adapts to change and competing priorities.
  • Ability to de‑escalate crisis situations with professionalism and sound judgment.
  • Possess and maintains a valid Drivers’ License, reliable vehicle, current registration and insurance.
  • Ability to effectively interact with clients and members of various teams within and outside the agency.

  • Understands and abides by all agency policies and procedures.
  • Completes and maintains documentation and required paperwork for all clients on time.
  • Manages all relationships on behalf of agency and clients in a professional and respectful manner.
  • Able to solve practical problems and deal with a variety of variables in situations with limited standardization.
  • Proficient with use of computer i.e. Microsoft Office, Electronic Health Record etc., filing skills and other administrative skills.
  • Skilled at individual and group based interventions.
  • Demonstrates excellent organizational, time management skills.
  • Effectively communicates and presents information clearly and in a concise manner in verbal and written format.
  • Demonstrates excellent interpersonal skills including professional demeanor and positive, service‑oriented attitude toward all internal and external customers.
  • Demonstrates ability to adhere to strict Confidentiality and HIPAA Guidelines.
  • Provides guidance, assistance and support to clients as well as works collaboratively with Chrysalis Team
