As a therapist at Center Well Home Health, reporting to the Home Health Branch Administration, you will play a vital role in helping patients regain strength, mobility and independence—all from the comfort of their homes. By delivering personalized care that focuses on rehabilitation and functional improvement, you will empower individuals to overcome physical limitations, perform everyday activities with confidence and enjoy a better quality of life.

Responsibilities
  • Assess and screen a patient’s daily living and work‑related skills, and develop therapeutic retraining programs with measurable objectives.
  • Administer and interpret diagnostic and prognostic tests of function in patients’ homes or other settings to assist physicians in evaluating levels of function.
  • Confer with physicians and other health‑care team members to develop and revise the plan of care and treatment.
  • Provide therapeutic treatment and instruction in accordance with physician orders to improve or restore strength, coordination, range of motion and function, or teach compensatory measures.
  • Review and expand the retraining program consistent with each patient’s behavioral goals and rehabilitation guidelines.
  • Consult with vocational team members to develop and implement vocationally oriented plans aligned with patients’ needs and capabilities.
  • Report patient status and progress to physicians, rehabilitation staff, clinical managers, patients and families.
  • Participate in care coordination and discharge‑planning activities; act as a resource for other health‑care personnel.
  • Design community reintegration activities to facilitate physical reconditioning or psychological adjustment, coordinating the plan with the interdisciplinary team.
  • Recommend or design adaptive equipment to improve residence or work environments or enhance participation in rehabilitation.
  • Design and train staff and family members in the retraining program, including dressing, feeding, grooming and hygiene skills, and participate in family educational meetings.
  • Supervise, instruct and evaluate occupational‑therapy assistants and home‑health aides in occupational‑therapy services.
  • Accurately and thoroughly document care observations, interventions and evaluations on days services are rendered.
  • Ensure interim verbal orders from physicians are documented and implemented correctly.
  • Submit evaluations, treatment plans and discharge summaries to supervisors and care‑management staff.
Qualifications
  • Required: Current and unrestricted OT licensure; current CPR certification; valid driver’s license, auto insurance and reliable transportation.
  • Preferred: Minimum six months of occupational therapy experience; home‑health experience.

Additional Information
  • TB Screening:
    This role is patient‑facing and part of Humana’s tuberculosis screening program; TB screening is required.
  • Driver Safety:
    This role is part of Humana’s driver‑safety program. A valid state driver’s license and personal vehicle liability insurance are required.
  • Weekly

    Hours:

    Full time (40 hours per week). Hours may vary by location and other factors.
  • Pay:
    Base pay range $85,400 – $117,500 per year, plus $54.00 – $76.00 per visit/unit.
