Home Health Licensed Vocational Nurse

About the Role

As a Center Well Home Health nurse, reporting to the Clinical Manager, you will provide personalized one‑on‑one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you will manage care plans that support recovery and help patients get back to the life they love.

• Provide skilled nursing care to patients in their homes under the direction of an RN.
• Implement individualized care plans in collaboration with the patient, family, and healthcare team.
• Educate patients and families on disease management, treatment options, and self‑care techniques.
• Maintain accurate records of patient care, including documentation of visits, interventions, and outcomes.
• Maintain contact with patients, physicians, clinical manager(s), and other team members promptly regarding patient or family needs and status changes; participate in care coordination activities and discharge planning.

• Current nursing license in the state of practice.
• Valid driver’s license, auto insurance, and reliable transportation.
• Current CPR certification.
• At least two years experience as an LPN/LVN in a clinical setting.

• Experience in a Home Health or Hospice setting.

Pay per visit/unit: $33.00 – $47.00.

Full‑time (40 hours per week) pay range: $53,000 – $73,000 per year.

TB Statement: This role is part of Humana's Tuberculosis screening program; screening is required if selected.

Driving Statement: This role is part of Humana's driver safety program. A valid state driver’s license and personal vehicle liability insurance meeting minimum state limits ($25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 property damage or higher) are required.
