The Licensed Practical Nurse (LPN) in Assisted Living plays a critical role in promoting resident health, safety, and well‑being. This role provides direct nursing care, monitors clinical conditions, ensures regulatory compliance, and collaborates closely with the Health & Wellness Director, physicians, families, and caregivers to create an exceptional resident care environment.

FLSA Status:
Hourly, Non-Exempt

Key Responsibilities

Conduct ongoing clinical assessments, including vitals, physical observations, and condition changes.

Document all clinical findings accurately in resident records and electronic systems.

Oversee medication administration, storage, documentation, and pharmacy communication.

Ensure compliance with state Assisted Living regulations and company policies.

Collaborate with physicians regarding resident conditions, treatment changes, and follow‑ups.

Communicate promptly with families about health status, updates, and concerns.

Assist with care plan development, updates, and interdisciplinary communication.

Support caregivers through on‑the‑floor coaching, skills reinforcement, and problem‑solving.

Participate in orientation and ongoing training of new team members.

Maintain nursing‑related employee and resident clinical files.

Respond to emergencies, including first aid, incident reporting, and follow‑up documentation.

Participate in on‑call rotations and provide after‑hours phone support.

Required to cover med pass and call‑offs as needed.

Regulatory & Compliance Responsibilities

Maintain familiarity with state Assisted Living rules, especially nursing and medication regulations.

Follow required infection control protocols, including PPE, sanitation, and outbreak response.

Ensure accuracy and completion of regulatory forms, audits, MAR reviews, and compliance reports.

Support survey readiness efforts and participate in corrective action plans when needed.

Qualifications

Education:

Active, unencumbered LPN/LVN license with required continuing education.

Experience:

Previous experience in Assisted Living, Memory Care, Skilled Nursing, or Long‑Term Care preferred.

Knowledge of senior living regulations and medication management best practices.

Strong communication and collaboration skills with residents, families, and staff.

Ability to perform physical tasks including lifting, transferring, and responding to resident emergencies.

Strong organizational skills and attention to detail.

Ability to prioritize tasks and manage time effectively.

Professional judgment in clinical decision‑making.

Ability to de‑escalate stressful situations and support families and residents emotionally.

Working Conditions

Fast‑paced Assisted Living/Memory Care environment with frequent resident interaction.

May require working weekends, holidays, and participation in on‑call rotations.

Exposure to residents with varying levels of cognitive and physical needs.

Stand and walk frequently during shifts, with tasks requiring bending, lifting, and assisting residents.

Work in environments where infection control and resident interaction are constant parts of the job.

May face stressful conditions due to resident needs and workload.

Can be exposed to communicable diseases and infections as part of daily duties.

May come into contact with bodily fluids.
