About This Role

The RN Care Manager at Cleveland Clinic Indian River Hospital will work collaboratively with a multidisciplinary care team to provide coordination of care and disease management for high-risk patients. This role is ideal for those passionate about promoting wellness and improving patient outcomes.

• Location: Vero Beach, FL
• Facility: Indian River Hospital
• Schedule: Full-time, Day shift

• Identify patients with ongoing care coordination needs for their specialty condition.
• Outline the nature and duration of involvement needed by the specialty care team.
• Utilize assessment skills and risk assessment tools to identify patients with care needs.
• Conduct targeted outreach to high-risk patients to ensure timely care delivery.
• Utilize technological tools to manage patient populations.
• Conduct comprehensive clinical assessments addressing various patient needs.
• Inform patients, families, and healthcare teams regarding care coordination.
• Develop goals and plan interventions to maximize patient outcomes.
• Monitor patient compliance with the plan of care.
• Serve as the primary patient contact for the care team.

• Graduate from an accredited school of professional Nursing (Diploma, ADN or BSN program) required.

• 3-5 years of nursing experience required.

• Current state licensure as a Registered Nurse (RN) required.
• Basic Cardiac Life Support (BLS) certification required.
• Specialty certification preferred.
