About This Role

The Care at Home Registered Nurse (RN) supports patients and families across the continuum of the medical home model. This role is ideal for compassionate caregivers who excel in delivering exceptional care and enhancing patient experiences.

• Location: Vero Beach, FL
• Facility: Indian River Hospital
• Schedule: Full-time, Night shift

• Provide direct nursing care.
• Implement, monitor, evaluate, update and revise patient care plans.
• Monitor, record, document and communicate patients’ conditions.
• Administer prescribed medications and treatments.
• Note and carry out physician and nursing orders.
• Assess and coordinate patients’ discharge needs.
• Educate patients and their families.
• Complete in-home assessments, monitoring, care planning, and interventions for acute care patients.

• Education: Graduate from an accredited school of professional nursing (ADN or BSN program)
• Experience: One year of current acute care RN experience; licensed military RN experience considered

  Basic Life Support (BLS) certification required

  Advanced Cardiac Life Support (ACLS) certification must be presented upon hire or within the first year of hire
