Job Title: RN Admissions Nurse Hospice

Unit: Hospice

SUMMARY

Provides skilled nursing assessment and determines eligibility for hospice services.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Assumes primary responsibility for initial patient/family education related to hospice election, services, and hospice coverage as it relates to the hospice diagnosis. Initiate communication with Hospice Medical Director, attending Physicians, Interdisciplinary group (IDG) and other agencies, needed to coordinate optimal care and use of resources for the patient/family and their immediate needs.
  • Provides information, pre‑assessment, and admission services to patients, conducting pre‑assessment in the home or health care facility as necessary.
  • Obtain up‑to‑date medical records, seek input from other team members regarding the patient plan of care, and obtain additional knowledge that supports hospice eligibility and end‑of‑life prognosis.
  • Completes the admission process with patients and families including explaining hospice services, insurance benefits, completing all required consents and required documentation in the EMR.
  • Establishes initial POC, medication profile, conducts medication review with Pharmacist and orders an initial DME.
  • Keeps Patient Care Manager and Admission Coordinator informed about progress of referral/admission or NTU.
  • Inform the RN Patient Care Manager of unusual or potentially problematic patient/family issues.
  • Meet regularly with Hospice nursing staff to review problems or unique issues, share professional support and exchange feedback aimed toward enhancing professional growth.
  • Share in providing 24‑hour, seven‑day week coverage to patient/families.
  • May be responsible to take after hours or weekend on call.
  • Will work one summer and one winter holiday.
  • Assume role of associate nurse during on‑call time or to fill patient/family needs during the absence of other primary nurses.
  • Provide appropriate support at time of death and period of bereavement if indicated.
  • Participate in VNA Hospice orientation and in‑service training programs for professional staff.
  • Participate in agency and community programs as requested, to promote professional growth and understanding of Hospice care.

Carriers out supervisory responsibilities in accordance with the organization’s policies and applicable law.

QUALIFICATIONS
  • Current RN license in the State of Florida
  • Minimum of 1 year experience as an RN in Hospice Care
  • Extensive knowledge of Hospice Conditions of Participation with Medicare
LANGUAGE SKILLS

Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
