Assistant Nurse Manager - Care Management

The Assistant Nurse Manager (ANM) in Care Management at Cleveland Clinic Indian River Hospital will utilize clinical expertise and understanding of care management and Medicare regulations to enhance patient care and ensure safe transitions. • Location: Vero Beach, FL
• Facility: Indian River Hospital
• Schedule: Full-time, Day shift

• Coordinate and facilitate patient care progression throughout the continuum.
• Collaborate with Social Work on the facilitation of transfers internally to SNF, ARF, LTACH, and Home Care.
• Maintain active communication with physicians, nursing, and other interdisciplinary team members for timely patient management.
• Assist in developing departmental practices and procedures for transitional care planning ensuring compliance with Medicare regulations.
• Analyze productivity and other outcome measures.
• Identify opportunities for continuous improvement.
• Manage information systems to maximize efficiency in review and transitional care planning.
• Develop and monitor routine data quality validation.
• Act as a liaison with other review-related and transitional care planning functions.

• Bachelor’s Degree in Nursing (BSN) required

• 2 years of recent experience in care management or transitional care planning
• 3 years of experience in hospital-based nursing

• Current and valid license to practice as a Registered Nurse (RN) in the State of Florida
• Care Management Certification required
