Job Overview

Graham Healthcare Group is hiring a Registered Nurse (RN) for a Hospice position providing end‑of‑life care in patient homes in the Monroeville, PA area. The role is ongoing, requires 40 hours per week with 8‑hour shifts, and offers a $10,000 first‑year success bonus.

• Deliver compassionate, patient‑centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life.
• Perform comprehensive patient assessments, symptom and pain management, and individualized end‑of‑life care planning.
• Educate and support patients and families through the end‑of‑life journey, ensuring they feel informed, prepared, and deeply cared for.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
• Work independently, while maintaining collaboration with the team.
• Travel to patient homes within an assigned territory.

• Active Registered Nurse license in the state of employment.
• At least two years of RN experience, with hospice experience preferred.
• Current driver’s license and ability to spend approximately 20% of the day driving to and from patient locations.
