Gentiva is seeking a dedicated Hospice RN / RN Case Manager to lead patient care in the home setting across Mississippi. You will act as the primary nurse for patients and families, building individualized plans of care and guiding them through the hospice journey with dignity, comfort, and respect.
You will perform initial and ongoing assessments, coordinate with physicians, LPNs, CNAs, social workers, chaplains, and other team members to ensure timely interventions and quality care.
