Clinical Navigator

• Work under the direction of the Director of Business Development
• Cultivate referral relationships
• Manage assigned territory
• Complete patient assessments
• Make sales contacts
• Analyze referral and admission data from the assigned territory
• Collaborate with business development team members, including the Admissions Manager and admission staff, nursing, and other internal and external staff to facilitate referral conversion
• Provide feedback and recommendations for program development and quality improvement initiatives related to customer service, referral and admission processes, and patient and referral source satisfaction
• Develop and maintain relationships with prospective patients, family members, physicians, ICU and floor nurses, discharge planners, case managers, and payer representatives

• Degree in a health-related field from an accredited college or university preferred
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required
• Minimum of two years' experience in the designated field of license
• Rehabilitation/LTACH and marketing experience preferred

Demonstrates expertise in patient assessments, referral management, and relationship cultivation within healthcare settings. Proven ability to analyze data for program development and quality improvement initiatives while collaborating effectively with multidisciplinary teams.
