Position OverviewThe Nurse Practitioner (NP) plays a critical clinical leadership role in delivering high‑quality hospice and palliative care. This position collaborates closely with physicians and the interdisciplinary team (IDT) to ensure patients and families receive comprehensive, compassionate end‑of‑life care.
Key Responsibilities- Collaborate with the IDT to evaluate and manage patient care needs and treatment plans
- Provide clinical guidance to ensure quality hospice and palliative care delivery
- Perform and document patient visits based on plan of care
- Conduct required face‑to‑face encounters for hospice eligibility and recertification
- Educate patients and families on disease progression, symptom management, and end‑of‑life care
- Communicate effectively with attending physicians and hospice medical director
- Monitor patient condition and report changes to appropriate team members
- Provide clinical education and support to staff as needed
- Participate in night and weekend on‑call rotation (Homecare & IPC)
- Maintain compliance with all regulatory, safety, and confidentiality standards
Education:
- Graduate of an accredited nursing program (Diploma, ADN, or BSN)
Licensure
- Current Texas RN license
- Current Advanced Practice Nurse (APN) license
- Valid driver's license
Experience
- Minimum 2 years of experience as a Nurse Practitioner
- Hospice or home health experience strongly preferred
Skills & Knowledge
- Strong clinical assessment and critical thinking skills
- Knowledge of pharmacology, palliative care, and end‑of‑life practices
- Excellent communication and interpersonal skills
Additional Requirements
- Reliable transportation and current auto liability insurance
- Ability to meet physical demands (lifting up to 50 lbs., prolonged standing/walking)
- Patient care provided in home and inpatient settings
- Exposure to various environmental conditions and weather
- Possible exposure to infectious diseases and bodily fluids
- Flexible schedule required to meet patient needs
