Altus Hospice is seeking a full-time Hospice RN Case Manager to join our nursing team serving Victoria and surrounding areas. You will deliver hands-on hospice nursing, coordinate services, and guide patients and families through the hospice journey.
The ideal candidate is a registered nurse with at least one year in medical-surgical/acute care, proficient with EMR, CPR certified, and holds a valid driver s license. We offer comprehensive benefits and a supportive onboarding program.
