JOB PURPOSE:
Provides intensive case management services to offenders with severe and persistent mental illness who are on adult criminal justice supervision (probation or parole) within our service area with the goal of reducing recidivism and successful completion of probation or parole through management of mental health related problems.
EDUCATION AND EXPERIENCE:Required
A bachelor's degree from an accredited college or university with a major in social, behavioral or human services or is a registered nurse. (Social, behavioral, and human services are psychology; social work; medicine; nursing; rehabilitation; counseling; sociology; human development; physician's assistant; gerontology; education psychology; special education; early childhood intervention; or early childhood education.)
PreferredAt least six months' experience in providing mental health services to adults, children and adolescents; work experience within jail setting.
DEGREE AND/OR CERTIFICATIONREQUIRED:
BA/BS in field specified above or licensed as a Registered Nurse in the State of Texas.
2.Provides Intensive Case Management services to offenders according to the recovery plan, TCOOMMIICM PGPs, and Medicaid Rule.
3.Ensure each offender receivesa minimum of3.5 face-to-face hours of service per month to include services provided by case manager, nursing, psychiatrist, and benefits specialists.
4.Provides ninety-five percent (95%) of intensive case management services in the consumers' natural environment.
5.Submits treatment recommendations through
Utilization Review based on medical necessity.
6.Documents services collaboratively according to Medicaid Rule andTCOOMMIPGP's.
7.Complete all paperwork according to Medicaid Rule and Center policy.
8.Submitsdocumentation of services within 2 business days of service delivery.
9.Worksin collaboration with the supervising officer to help stabilize mental health symptoms and reduce the risk of recidivism.
10.Facilitatesquarterly Interdisciplinary Team meetings with offender, Case Manager, supervisingofficer, and program director/designee.
11.Submitsincident reports to TCOOMMI within contract required time frames.

Responsible for completing assessments, recovery planning, and effective coordination of appropriate community services. Works closely with the criminal justicesystem and acts as liaison between offenders and supervising officers. Requires travel within the service counties.
Meets multiple deadlines and reporting requirements. Services will be provided in the community or clinic and will include at least one monthlycontact with the assigned supervising officer. - Task management:
Ability to perform tasks effectively and efficiently and to handle multiple priorities in a confidential environment. Must have strong attention to detail. Ability to work collaboratively with a team of staff to accomplish the efficient operation of the program by assisting coworkers, as necessary, to meet client needs and contract requirements with no verified complaints. Ability to work cooperatively and productivelywith supervisors, individuals, co-workers, and groups of people at all levelsof activity, contributing to a strong spirit of teamwork.
