The RoleAs a Nurse Practitioner at Greater Good Health, you will play a key role in providing exceptional care to our Medicare‑Medicaid dual eligible population. You will dedicate time to address patient needs and concerns, working closely with a multidisciplinary team.
Responsibilities- Manage patients with chronic conditions including diabetes, hypertension, CKD, and others.
- Perform comprehensive health assessments and conduct scheduled follow‑up visits to assess symptoms, review labs and vitals, and adjust treatment plans, ensuring accurate clinical coding and documentation.
- Provide patients with education on preventive measures, lifestyle management, and disease management.
- Support highly complex patients, assisting them in navigating a challenging healthcare system and delivering one‑on‑one care they need and deserve.
- Operate both autonomously and collaboratively with all members of the healthcare team to coordinate and facilitate high‑quality, cost‑effective care.
- Coordinate care services among the patient, family, physicians, additional providers, and other care team members.
- Administer various tests and screenings.
- Maintain strict confidentiality in line with HIPAA regulations.
This is a part‑time Nurse Practitioner opportunity (3 days a week, 9am‑6pm PST schedule). The role may include a mix of in‑clinic, in‑home visits primarily within a 30‑mile service area, and telehealth appointments. Flexibility across regions may be required to provide coverage as needed.
Experience and Qualifications- Nurse Practitioner certification from ANCC or AANP.
- 2+ years of clinical experience as an NP in Primary Care, Family Medicine, Internal Medicine, Gerontology, or Palliative Care (home care experience is a plus).
- Working knowledge of chronic care management and medication options.
- Current DEA licensure / prescriptive authority post‑hire.
- Active CPR/BLS certification.
- Knowledge/history of working with HCCs.
- Experience working with Medi-Medi/dual‑eligible or underserved populations preferred.
- Bilingual (English/Spanish) is required
- Other duties as assigned.
- License reimbursements for conditions of employment (DEA, license, board certification, ALS/BLS).
- Collaborative and supportive community with dedicated care coordinators and MD advisors.
- Malpractice coverage for peace of mind.
Salary range prorated for 3 days/week.
