RoleAs a Nurse Practitioner at Greater Good Health, you will help provide exceptional care to our Medicare‑Medicaid dual‑eligible population. Your responsibilities include:
- Managing patients with chronic conditions such as diabetes, hypertension, CKD, and more.
- Performing comprehensive health assessments and scheduled follow‑up visits to review symptoms, labs, vitals, and adjust treatment plans.
- Providing education on preventive measures, lifestyle management, and disease management.
- Supporting highly complex patients by navigating the health system and delivering individualized care.
- Co‑ordinating care both autonomously and collaboratively with the healthcare team for cost‑effective, high‑quality service.
- Administering various tests and screenings as needed.
- Maintaining strict confidentiality in line with HIPAA regulations.
This full‑time Nurse Practitioner opportunity combines in‑clinic, in‑home visits within a 30‑mile service area, and telehealth appointments. Flexibility to cover regions as scheduling needs arise is required.
Experience and Qualifications- Nurse Practitioner certification from the ANCC or AANP.
- 2+ years of clinical experience as an NP in Primary Care, Family Medicine, Internal Medicine, Gerontology, or Palliative Care. Home care experience is a plus.
- Working knowledge of chronic care management and medication options.
- Current California Nurse Practitioner license.
- Board‑certified NP in good standing.
- Current DEA licensure / Prescriptive Authority post‑hire.
- Active CPR/BLS certification.
- Knowledge/history of working with HCCs.
- Experience with Medi‑Medi / dual‑eligible or underserved populations preferred.
- Bilingual (English/Spanish) required.
- Other duties as assigned.
