ResponsibilitiesThe Quality Coordinator (QC) works closely with the Quality Manager, Medical Director and Administrator to implement quality platforms. This includes chronic care management (CCM), transitional care management (TCM) and other quality initiatives such as HEDIS, P4P, MIPS, STAR measures and annual wellness visits. The QC assesses missing patient benchmarks and organizes data in preparation for outreach or encounters with healthcare providers.
Through mailing letters, sending educational materials, making calls to patients or providers, and creating referrals, the QC contributes to patient satisfaction and quality outcomes. The QC provides general education related to missing or due quality measures to providers, reviews gaps in care reports, and educates providers to close data gaps. The QC also provides backup to the Member Services Representative and handles other duties as assigned by the Quality Manager.
