Must-Have:
Recent home health experience is required.
Reliable transportation with a current driver's license and agency-required liability insurance.
Willingness to travel up to 75 miles for home visits.

Description:

Home visits will be performed for therapy.
Periodic office meetings will be required.
The average number of patients per day is between 5 to 10.
Each type of visit and activity will have a different point value, with expectations of hitting a weekly productivity target.
Documentation and travel activities will contribute to point totals.
Contractors must accept every patient scheduled, regardless of distance from previous appointments.
All documentation must be completed within 24 hours of the patient appointment.
Failure to meet documentation and productivity expectations may result in canceled shifts.
There are no guaranteed hours with this contract.
Mileage will be reimbursed at the standard IRS rate for travel between patient homes.
The commute to and from the first and last patient is not billable.
Local candidates are accepted.
Must have a vehicle and auto insurance.
The position may require coverage for home health visits in patients' homes and assisted living facilities.
Candidates should be comfortable with potentially long drives between patients.

Experience with specified EMR systems is preferred.
No on-call duties are required.
Candidates must be open to floating to nearby locations if low patient census affects hours.
Home health experience is preferred.
