Seven Healthcare is excited to present an outstanding opportunity for a Case Management Registered Nurse in Vidalia, Georgia. This 13‑week travel contract includes a weekly blended rate of $2,600, based on five 8‑hour day shifts. This role is ideal for a Case Manager who is passionate about helping patients navigate their care safely and efficiently.

• Conduct patient assessments focused on discharge readiness and resource needs
• Plan and coordinate safe transitions across all post‑acute settings
• Perform utilization reviews and communicate with insurance providers
• Communicate care plans clearly to patients, families, and multidisciplinary partners
• Maintain accurate, detailed records in the EMR
• Advocate for patients through complex medical and psychosocial situations
• Support continuity, safety, and quality of care throughout the patient stay

• Active Georgia RN license or compact eligibility
• Current BLS certification
• Minimum 1–2 years of Case Management experience
• Strong communication and critical‑thinking abilities
